About the Role:
We are looking for a dedicated Care Coordinator to ensure that clients receive high-quality care in their homes while leading independent and fulfilling lives. You will play a crucial role in managing care schedules, communicating with health professionals, and ensuring a smooth care service that meets CQC standards.
This role requires excellent communication and organizational skills, with a willingness to provide direct client support when needed. You will work weekdays with occasional after-hours and weekend on-call duties.
Key Responsibilities
✔️ Schedule and manage care staff rotas
✔️ Communicate rota changes effectively to clients and colleagues
✔️ Maintain up-to-date care records and electronic documentation
✔️ Ensure compliance with safety procedures and company policies
✔️ Build strong relationships with clients, families, and healthcare professionals
✔️ Monitor care packages and conduct evaluations
✔️ Participate in the emergency on-call roster
Requirements
🔹 Experience in healthcare or social care
🔹 Strong organizational and decision-making skills
🔹 Ability to work under pressure and solve problems
🔹 Self-motivated with a passion for care
🔹 UK driver’s license and access to a vehicle (required)
🔹 Ability to provide references and pass a DBS check
🔹 Proficiency in Microsoft Office (Word, Excel, Outlook, etc.)
Work Schedule
